حب الشبـــــــــــــــــاب

هناك عدة عوامل تتكامل وتؤدى إلى ظهور حب الشباب أهمهاالعوامل الهرمونية والميكروبية والوراثية :

- العوامل الهرمونية :

إن وجود غدد دهنية نشيطة شرط أساسى لظهور حب الشباب الذى يصيب الغدد الدهنية النشيطة فقط وقد ثبت علميا أن الهرمون الذكرى وهو الأندروجين هو الهرمون المنبه للغدد الدهنية فى الذكور وفى الإناث أيضا ومصادره هى الخصيتان فى الذكور والمبيضان والغدة فوق الكلوية فى الإناث.

ومن الملاحظ أن حب الشباب لا يصيب المرضى الذين يعانون من نقص فى إفراز هرمون الذكورة بينما يبدأ فى الظهور بعد بدء العلاج التعويضى بذلك الهرمون ويلاحظ انتكاس المرض فى الفتيات مع كل دورة طمثية .

- العوامل الميكروبية :
تعيش بعض الميكروبات بصفة طبيعية على سطح الجلد وفى فوهات الجريبات الشعرية الدهنية وهى تتعايش مع الإنسان ولا تسبب له متاعب فى العادة ويعيش أحد تلك الميكروبات داخل فوهة الغدة الدهنية ويتكاثر مع زيادة الإفرازات الدهنية وينتج ذلك الميكروب عند تكاثره مواد التهابية تنفذ من الجريبة فتسبب التهابات حولها.

- العوامل الوراثية :
لوحظ فى كثير من الحالات أن المصاب بحب الشباب ينحدر من أبوين كان قد أصيب أحدهما أو كلاهما المرض كما لوحظ وجود استعداد خاص للإصابة بالمرض لدى بعض الأسر .

- العوامل الغذائية :
لم يثبت وجود علاقة بين الإكثار من تناول المواد الدهنية والمأكولات الدسمة والشيكولاتة وظهور أو زيادة حدة حب الشباب - كما كان يعتقد فى الماضى لذلك لا داعى لامتناع مرضى حب الشباب عن تناول تلك الأصناف .

- حب الشباب
يظهر على المناطق المصابة مثل الوجه والجذع والذراعيين وهو خليط من أشكال عديدة مثل :
الرأس السوداء وهى الكوميدون وتظهر على هيئة نقطة سوداء فإذا ضغط عليها برزت على شكل دويدة بيضاء يميل لونها للصفرة ذات رأس أسود وهناك أيضا نوع أصغر يسمى الرأس البيضاء .
1. حبيبات حمراء صغيرة
2. بثرات صديدية صفراء تظهر على رأس الحبيبات
3. خراج مؤلمة
4. أكياس عميقة

وفى أغلب الأحيان تتألف الإصابة من أكثر من شكل من الأشكال السابق ذكرها وتزول الإصابات بعد فترة تتراوح بين أيام وأسابيع ليظهر غيرها ويمر بنفس المراحل ويعقب زوال الإصابة البسيطة السطحية مثل الحبوب والبثرات السطحية ظهور بقع حمراء تستمر لعدة أسابيع ثم تختفى دون أثر يذكر . أما فى الإصابات الشديدة العميقة فيلاحظ بقاء ندبات محفورة مشوهة لا تختفى إلى الأبد . وبشكل عام يمكن تقسيم الإصابة بحب الشباب إلى نوعين رئيسيين : النوع البسيط والذى يتميز بظهور رؤوس سوداء أو حبيبات وبثرات سطحية ولا يخلف ذلك النوع آثارا مستديمة عند زواله والنوع الشديد ويتميز بظهور بثرات عميقة وخراريج وأكياس تترك ندبات محفورة مشوهة بعد زوالها

التوحد هو حالة من حالات الإعاقة التى لها تطوراتها، وتعوق بشكل كبير طريقة استيعاب المخ للمعلومات ومعالجتها. كما أنها تؤدى إلي مشاكل في اتصال الفرد بمن حوله، واضطرابات في اكتساب مهارات التعلم والسلوك الاجتماعى. لا يوجد سبب معروف لهذا النوع من الإعاقة. لا توجد طريقة أو دواء بمفرده يساعد فى علاج حالات التوحد، لكن هناك مجموعة من الحلول مجتمعة مع بعضها اكتشفتها عائلات الأطفال المرضى والمتخصصون.
وتظهر إعاقة التوحد بشكل نمطى خلال الثلاث سنوات الأولى من عمر الطفل لكل حوالى 15-20مولودآ / 10.000، وتفوق نسبة إصابة الصبية أربع مرات نسبة إصابة البنات.ويحيا الأشخاص المصابون بهذا النوع من الإعاقة حياة طبيعية وتجدها منتشرة في جميع بلدان العالم وبين كل العائلات بجميع طوائفها العرقية والاجتماعية.

الأسباب التى تؤدى إلى التوحد:
لا يوجد سبب معروف لهذا النوع من الإعاقة، لكن الأبحاث الحالية تربطه:
1- بالاختلافات البيولوجية والعصبية للمخ. لكن الأعراض التى تصل إلى حد العجز وعدم المقدرة علي التحكم في السلوك والتصرفات يكون سببها خلل ما في أحد أجزاء المخ.
2- أو أنه يرجع ذلك إلى أسباب جينية، لكنه لم يحدد الجين الذي يرتبط بهذه الإعاقة بشكل مباشر.
3- كما أن العوامل التى تتصل بالبيئة النفسية للطفل لم يثبت أنها تسبب هذا النوع من الإعاقة.
4- تناول العقاقير الضارة أثناء الحمل لها تأثير أيضاً.
5- وهناك جدل آخر حول العلاقة بين فاكسين (إم.إم.آر) والإصابة بإعاقة التوحد.
6- وقد يولد الطفل به أو تتوافر لديه العوامل التى تساعد على إصابته به بعد الولادة ولا يرجع إلى عدم العناية من جانب الآباء.
7- ويظهر التوحد بين هؤلاء الذين يعانون من مشاكل صحية أخرى مثل:
Fragile X Syndrome
Tuberous Sclerosis
Congential Rubella Syndrome
Phenylketonuria

الأعراض:
علامات الإصابة بإعاقة التوحد:
- الكلام في الحديث مكرر ومتكلف.
- الصوت يكون غير معبرآ أو يعكس أياً من الحالات الوجدانية أو العاطفية.
- تمركز الحديث عن النفس.

تشخيص التوحد:
لا توجد اختبارات طبية لتشخيص حالات التوحد، ويعتمد التشخيص الدقيق الوحيد على الملاحظة المباشرة لسلوك الفرد وعلاقاته بالآخرين ومعدلات نموه. ولا مانع من اللجوء فى بعض الأحيان إلى الاختبارات الطبية لأن هناك العديد من الأنماط السلوكية يشترك فيها التوحد مع الاضطرابات السلوكية الأخرى. ولا يكفى السلوك بمفرده وإنما مراحل نمو الطفل الطبيعية هامة للغاية فقد يعانى أطفال التوحد من:
1- تخلف عقلى.
2- اضطراب فى التصرفات.
3- مشاكل فى السمع.
4- سلوك فظ.

أدوات التشخيص:
يبدأ التشخيص المبكر وذلك بملاحظة الطفل من سن 24 شهراً حتى ستة أعوام وليس قبل ذلك، وأول هذه الأدوات:
1- أسئلة الأطباء للآباء عما إذا كان طفلهم:
- لم يتفوه بأيه أصوات كلامية حتى ولوغير مفهومة فى سن 12 شهراً.
- لم تنمو عنده المهارات الحركية ( الإشارة- التلويح باليد - إمساك الأشياء) فى سن 12 شهراً.
- لم ينطق كلمات فردية فى سن 16 شهراً.
- لم ينطق جملة مكونة من كلمتين فى سن 24 شهراً.
- عدم اكتمال المهارات اللغوية والاجتماعية فى مراحلها الطبيعية. لكن هذا لا يعنى فى ظل عدم توافرها أن الطفل يعانى من التوحد، لأنه لابد وأن تكون هناك تقييمات من جانب متخصصين فى مجال الأعصاب، الأطفال، الطب النفسى، التخاطب، التعليم.

2- مقياس مستويات التوحد لدى الأطفال(Cars):
ينسب إلى "إيريك سكوبلر " Eric schopler - فى أوائل السبعينات ويعتمد على ملاحظة سلوك الطفل بمؤشر به 15 درجة ويقييم المتخصصون سلوك الطفل من خلال:
- علاقته بالناس.
- التعبير الجسدى .
- التكيف مع التغيير.
- استجابة الاستماع لغيره.
- الاتصال الشفهى.

3- قائمة التوحد للأطفال عند 18 شهراً (Chat):
تنسب إلى العالم"سيمون بارون كوهين" Simon Baron-Cohen - فى أوائل التسعينات وهى لاكتشاف ما إذا كان يمكن معرفة هذه الإعاقة فى سن 18 شهراً، ومن خلالها توجه أسئلة قصيرة من قسمين القسم الأول يعده الآباء والثانى من قبل الطبيب المعالج.

4- استطلاع التوحد:
وهو مكون من 40 سؤالاً لاختبار الأطفال من سن 4 أعوام وما يزيد على ذلك لتقييم مهارات الاتصال والتفاعل الإجتماعى.

5- اختبار التوحد للأطفال فى سن عامين:
وضعه "ويندى ستون" Wendy Stone - يستخدم فيه الملاحظة المباشرة للأطفال تحت سن عامين على ثلاث مستويات التى تتضح فى حالات التوحد : اللعب - التقليد (قيادة السيارة أو الدراجات البخارية) - الانتباه المشترك.

The uvula is located at the back of your throat which hangs as a soft palate, made of connective tissues having a number of glands, and muscular fibers. The uvula helps in the articulation of the sound of human voice. It works together as one with the back of the throat, the palate, and air which is passed from the lungs to make sounds.

However, the uvula can be a reason for snoring or ear-ringing sounds when you sleep especially when it’s elongated. It causes more vibrations which may produce eerie sounds. Also, an elongated uvula can cause sleep apnea which literally means “want of breath” , because it can block the passage of air from the lungs to the throat. However, medical experts say snoring and sleep apnea can be treated by removing a part of the uvula or the whole of it in an operation called as uvulopalatopharyngoplasty.

But the success of this operation is now wholly guaranteed because the snoring and sleep apnea can come back and may even tend to worsen.

This little 12-year old boy from Norway has probably earned excuse to play World of Warcraft for the rest of his life. 12-year old Hans Jorgen Olsen and his sisters were walking in the woods near his home when they stumbled upon an angry moose which obviously didn’t welcome the ‘intruders’ and headed towards his sister. He suddenly remembered some skills he learned while playing WOW so he decided to ‘taunt’ the animal.

After animal was provoked by him and headed in his direction he used another skill from WOW, feigning the death. Moose approached him and believed that he is dead and suddenly lost an interest for hurting him. After moose left, they managed to get back home safely.

يقول علماء ان الخلود الى النوم بعد تعلم شيئ جديد قد يساعد على ترسيخ المعلومات المكتسبة في الدماغ، شريطة ان يرى الشخص احلاما في منامه.

واكتشف الباحثون ان من يبقى مستفيقا بعدما يلقن شيئا جديدا يظهر فيما بعد قدرة استيعاب اقل ممن تمتعوا بقيلولة او رأوا احلاما في منامهم.

وطلب من المشاركين في الدراسة حفظ تفاصيل متاهة على جهاز كمبيوتر على ان يتذكروا الطريق الى مخرجها بعد بضع ساعات.

ووجد اولئك الذين ذهبوا في قيلولة واولئك الذين تذكروا رؤية حلم خلالها طريق الخروج من المتاهة اسرع من غيرهم.

ويعتقد الباحثون ان الاحلام علامة على ان الدماغ يبذل جهدا كبيرا لمعالجة المعلومات التي تلقاها.

ويقول احد القيمين على الدراسة الدكتور روبرت ستيكجولد من معهد هارفرد الطبي ان الاحلام قد تكون علامة على كون الدماغ يدرس المعلومات المتلقاة على اكثر من مستوى.

واضاف: "وقد يكون الدماغ يحاول ربط المعلومات بغيرها لتسهيل حفظها واستخدامها في تطبيقات مختلفة في المستقبل."

وحسب زميلته ايرين وامزلي، فان الدراسة تبين ان الدماغ يحاول التمسك بما يعتبره الاهم من بين المعلومات المكتسبة لان الفرد يلاقي ويجمع كما ضخما من المعلومات المختلفة كل يوم.

وتقول وامزلي: "يبدو ان دماغنا يتساءل عندما ننام: كيف استفيد مكن هذه المعلومات وماذا انتقي منها؟"

ويقول الفريق الباحث انه قد يجد تطبيقات عملية لهذه الدراسة في سبيل دعم الذاكرة وتحسين طرق التعلم.

ومن بين ما يعتقدون ان الطلبة قد يستفيدون اكثر ان هم تعلموا شيئا جديدا قبل الخلود الى النوم او قبل قيلولة.

أظهرت دراسة جديدة لعلماء المعهد الوطني الأمريكي للسرطان أن تعرض الأطفال إلى دخان السجائر و هو ما يعرف باسم التدخين السلبي يؤدى لارتفاع خطر إصابتهم بسرطان الرئة خلال حياتهم فيما بعد حتى و أن لم يدخنوا السجائر .

و تضمنت الدراسة الجديدة 2000 فرد حيث تم فحص تأثير التدخين السلبي على جين معين بالجسم يسمى ( MBL2 ) .

و اكتشف العلماء علاقة بين التدخين السلبي في مرحلة الطفولة و الإصابة بسرطان الرئة فيما بعد خلال مرحلة الشباب حتى و أن لم يدخن الفرد كذلك ارتفاع نشاط جين ( MBL2 ) و الذي يرتبط أيضا بالإصابة بأمراض الرئة .

و يقول العلماء أن الدراسة تضيف المزيد من التحذيرات إلى مخاطر تعرض الأطفال إلى دخان السجائر و ما يسببه من أضرار كبيرة على صحتهم سواء بمرحلة الطفولة أو فيما بعد .

Swimming pools may be dirtier than most people think: the Centers for Disease Control and Prevention said this week more than 12 percent of pool inspections found violations serious enough to cause them to be immediately shut down.

The CDC analyzed data from at least 121,000 pool inspections in 13 states during 2008, according to ABC News.
